Job Description:

Physical Therapist PT opportunity for full time home health visits in Concord NH servicing adult and geriatric clients in Keene NH area. One year prior clinical experience required.
As a home care Physical Therapist you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing to clients allowing them to receive medical care at home.
Responsibilities include:

• 
  
• Make home visits to clients in designated territories.
  
• Perform diagnostic tests and measurements including mobility range of joints transfer status and safety assessments.
  
• Implement integumentary protective interventions to prevent wound formation and promote wound repair.
  
• Develop and implement individualized care plans including manual therapeutic exercises gait training balance and other interventions.
  
• Educate and instruct clients and family members in rehabilitative care and activities to promote health safety and independent living.
  
• Accurately document observations interventions and evaluations pertaining to client care management and services provided.
  
  Qualifications include:
  
  • 
    
  • New Hampshire Physical Therapist license.
    
  • Minimum one year of recent work experience as a Physical Therapist.
    
  • Graduation from a program approved by the American Physical Therapy Association.
    
  • Ability to work independently and manage time effectively.
    
  • Strong interpersonal organizational and problem solving skills.
    
  • Solid computer skills and prior experience with electronic medical records preferred.
